Incident to closure — the full lifecycle

From field report through investigation, notification, corrective actions, and verified closure — the exact motion an incident takes through WorkSafe.

An incident that never closes is worse than one that never happened — it means your program can't demonstrate corrective action. WorkSafe treats every incident as an accountable workflow — a named owner, a full audit trail, and a verified close.

1. Report from the field

  • A worker, lead, or admin taps Report Incident — from the header, from a project, or from the QR portal.
  • The form captures type, severity, description, injured party, witnesses, photos, location, and the WCB-reportable flag.
  • Submission works offline; queued and synced on reconnect.

2. Classification and routing

Incident type comes from the per-company incident_type_configs. Common types: First Aid, Medical Aid, Lost Time, Near Miss, Property Damage, Environmental.

  • If the type is WCB-reportable (or the flag was set manually), email alerts fire to the configured recipients.
  • A WorkSkedge task lands on the safety manager.
  • A reportable badge shows on the incident record.

3. Investigation

The safety manager or project lead opens the incident, adds investigation notes, root cause analysis, and any additional photos or witness statements. Site workers see only incidents they are directly involved in; leads see everything on their projects.

4. Corrective actions

  • Each incident spawns one or more corrective actions — owner, due date, priority from the risk matrix, description.
  • Required photo evidence on completion where the template mandates it.
  • Deep-link push to the assignee's WorkSkedge task inbox.
  • Corrective actions can also be raised independently — from a toolbox talk, a site inspection, or ad hoc.

5. Completion

  • Assignee marks the CA complete with photo evidence.
  • Final closure is restricted to admins, safety managers, or the project lead — this prevents self-closure.
  • Closure auto-completes the WorkSkedge task.

6. Reporting

  • Corrective Action Tracker report shows open, overdue, and closed CAs.
  • Incident Log report shows every incident with type, severity, WCB flag, and CA status.
  • Incident Cost report captures direct and indirect cost tracking.
